Aboriginal and Torres Strait Islander Clinic

Greenbank Pioneer Health Service 715 Health Clinic
The 715 Health Assessments for Aboriginal and Torres Strait Islander people covers the full age spectrum. The health assessment includes an assessment of the patient’s physical, psychological and social wellbeing. It also assesses what preventative health care, education and other assistance should be offered to the patient to improve health and wellbeing.
Upon registration of the Closing the Gap program in April 2013, Greenbank Pioneer Health has worked closely with the program to conduct 715 Health Clinics for Aboriginal and Torres Strait Islander patients accessing the practice. During September and October, Dr Rajeev Chhaya successfully completed a significant number of 715 Health Assessments with the assistance of Erica, the Practice Nurse.
During the clinic times, GMSBML’s Aboriginal and Torres Strait Islander Outreach Workers attended Greenbank Pioneer Health to support Dr Rajeev and Nurse Erica. Assistance provided was an explanation to patients about the Closing the Gap Program, the importance of 715 health assessments and referral pathways and services for patients to access. Overall, the clinics offered by Greenbank Pioneer Health have been extremely successful in supporting Aboriginal and Torres Strait Islander patients to access health services tailored to their needs.
